Class Disclaimer/Rules policy
As the person, or responsible party for minor, I am registering as a participant in an Art Salvage class.
I understand, agree and acknowledge:
Pre-registration is required for Art Salvage classes; classes will be filled on a first-come, first-served basis. Students will be registered ONLY on receipt of payment. Enrollment requirements: 3 students minimum. Classes with low enrollment are cancelled 7 days before the class is scheduled to begin.
Cancellation Policy: Art Salvage reserves the right to cancel, combine or reschedule classes to adjust for enrollment. If Art Salvage cancels a class, you may transfer to another class or receive a full refund.
Refund Policy: Notice of withdrawal from a class must be made at least 7 days prior to the start of the class. No refunds will be given for withdrawals made after that time. No refunds or credits will be given for classes missed due to illness, personal conflict or other reasons. There are no exceptions to this policy.
I agree to grant Art Salvage and its authorized representatives permission to photograph and/or video, pictures of my participation. I further agree that any or all of the material photographed may be used, in any form, as part of any future publications, brochure, or other printed materials used to promote Art Salvage, and further that such use shall be without payment of fees, royalties, special credit or other compensation.
If I DO NOT grant permission for me and/or my child's photograph and/or video to be recorded I will contact Art Salvage at email@example.com.
WAIVER OF LIABILITY
I, and my heirs and representatives, release and indemnify Art Salvage, its individual members and Board of Directors, teachers, volunteers and agents from all and against all and any actions or claims which may be made by me, or on my behalf by other parties or in respect to or arising out of injury, loss, damage or death caused to me or my property whether by negligence, breach of contract or in any other actionable capacity whatsoever.
I also agree that in the event that I or my child is injured or my property is damaged I, or my heirs and representatives, will bring no claim, lawsuit or other action, against Art Salvage, its individual Board of Directors, teachers, volunteers and agents in respect to that injury or damage.
I give my permission for the registered participant to receive emergency medical treatment if necessary.
All minors must be accompanied by an authorized adult at all times during Art Salvage class, unless otherwise directed by Art Salvage.
Please do not leave valuables unattended. Art Salvage will not be responsible for items lost, stolen, misplaced, damaged or left behind.
All participants are to wear attire and shoes that are appropriate for the class and that you would allow to become permanently damaged due to class activity.
Alcohol, drugs or weapons are NOT allowed on Art Salvage premises. No smoking or vaping are allowed on the property at any time.
I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any Art Salvage class. I represent and warrant that I have no medical condition that would prevent my participation in an Art Salvage class.
Inappropriate behaviour will not be tolerated. Art Salvage reserves the right to dismiss participants who do not comport their behavior to the instructions of Art Salvage, or violate any law or regulation.
